- A referral letter from your GP, handed in when you meet, sent through the encrypted upload link we provide or, with your consent, sent to us by your GP through Healthmail. Without one, your GP’s details are enough.
- The Pre-Assessment Form, downloaded here and completed at home (see below).
- Your insurance: check what is covered. Receipts are issued for your claim and for the 20% tax relief.
- Your medication list and any previous reports; for ADHD, school reports if available.
- Someone with you, if you wish. Lighting, seating and breaks can be adjusted if you let us know in advance.
- Getting there: a short walk from Dún Laoghaire DART station; parking in the town is paid. The rooms are on two floors — tell us when you book if stairs are a difficulty.
- Video appointments: a link arrives by email; you need a quiet, private space and a phone, tablet or computer. Consultations are never recorded; with your agreement, they are transcribed by Heidi, a medical AI scribe, to help Dr Szigeti draft his notes.
The forms
Every new patient completes the Pre-Assessment Form before the first appointment: short notes are fine, and anything left blank can be discussed when you meet. Completing it at home saves time during the appointment and helps Dr Szigeti tailor your care from the first meeting.

Return it by hand when you meet, or through the encrypted upload link we send you — not by email or WhatsApp. Depending on your assessment, you may also receive a consent form, a short questionnaire for someone who knows you well, and a health screening questionnaire before any ADHD medication.
Sending documents safely
Forms and referral letters never travel by ordinary email or WhatsApp: hand them in when you meet, or use the encrypted upload link we send you when your appointment is arranged. The link opens a secure upload page run by Tresorit, a Swiss company: files are encrypted on your own device before they leave it (end-to-end encryption), stored in data centres in the European Union, and only Dr Szigeti can open them — not even Tresorit.
